Northwestern Memorial HealthCare has told us they are leaving our network.

The termination includes the following medical providers: Northwestern Memorial Hospital & Physicians, Northwestern Lake Forest Hospital & Physicians, Northwestern Medicine Central DuPage Hospital & Physicians, Northwestern Medicine Delnor Hospital & Physicians, Marianjoy Rehabilitation Hospital, Northwestern Medicine Kishwaukee Hospital & Physicians, Northwestern Medicine Valley West Community Hospital & Physicians, Northwestern Medicine Huntley Hospital & Physicians, Northwestern Medicine McHenry Hospital & Physicians, Northwestern Medicine Woodstock Hospital, Northwestern Medicine Regional Medical Group, Marianjoy Medical Group, Northwestern Medical Group, and Northwestern Home Infusion and Pharmacy.

Services received from these medical providers will be considered out-of-network on July 1, 2021.
