Welcome to the HealthLink participating Provider Locator! It’s
simple; just follow sections 1-2-3 below to locate a provider or other health care professional in a HealthLink network. For searching
tips, place your mouse cursor over the title of a button, field or menu option and a brief description will appear
to help guide you through your search.
Create Customized Directory! Enter your search criteria
by completing sections 1-2-3 below. Once your results are displayed, click on the "Create Customized Directory"
button. You can view, print or save your PDF customized directories instantly.
Step 1: Select a Network
Select a network from the drop down menu listed to the right. If you’re not sure what network access you have, check the
back of your member ID card.
Please Note: Participating providers residing in northern Illinois, including the Chicago area, are Tier II providers. Please refer to your plan documents or consult your plan administrator for coverage details.
Step 2: Choose a Provider Type
A Physician
or other Healthcare Professional
Looking for a Physician This option allows you to search for individual physicians and other health care professionals. You can limit your search by specialty, last name, gender, name of the group they belong to, or a hospital they are affiliated with.
A Group of Physicians
Looking for a Group of Physicians This option allows you to locate physicians belonging to the same physician group. You must enter the name of the physician group into the name field in order to use this search option.
A Hospital
Looking for a Hospital This option allows you to locate a hospital. If you do not know the name of the hospital you are searching for, you may leave the name field blank in order to return a list of all hospitals.
An Ancillary Facility
Looking for an Ancillary Facility This option allows you to locate an ancillary facility based on ancillary type. You may select a specific ancilary type from the drop-down menu or leave it set to any to return all ancillaries.
List all Physicians, Hospitals, and Ancillary Facilities (Perfect when creating a Customized Directory)
List all Physicians, Hospitals, and Ancillary Facilities This option allows you to have a comprehensive listing of all physicians, hospitals, and ancillary facilities within a specific network returned to you based on geographic location.
With the Specialty: (Optional)
Specialty Option A Specialty is a physician devoted to a particular field of medicine. (i.e. Dermatology or Endocrinology). HealthLink categorizes PCPs (Personal Care Physicians) as Family Practice, General Practice, Internal Medicine, or Pediatrics. Please choose one specialty from the drop-down menu. This field is optional when searching for a physician.
With the Name: (Optional)
Name Option Enter the last name only of the physician you are searching for when performing a physician search. Enter the name of the physician group you are searching for when performing a group search. Enter the name of the hospital you are searching for when performing a hospital search. This field is optional when performing all searches except for the physician group search. For accurate search results, it is important to spell the name correctly. The HealthLink Locator is programmed to search partial words, but may not display accurate results if the spelling is incorrect.
Note:If searching for a physician, please enter their last name only. Note: If searching for a physician, enter the first letter of a physician’s last name to see an alphabetical
list of ALL physicians whose names begin with that letter.
Gender: (Optional)
Gender Option If you have a gender preference of the doctor you are searching for choose the appropriate gender. If you do not have a preference select Any Gender. This field is optional when searching for a physician.
Who Belongs to the Group: (Optional)
Physician Group Option Enter the name of a physician group. For accurate search results, it is important to spell the name of the group correctly. The HealthLink Locator is programmed to search partial words, but may not display accurate results if the spelling is incorrect. This field is optional when searching for a physician.
Is Affiliated with the Hospital: (Optional)
Hospital Affiliation Option Enter the name of a hospital. For accurate search results, it is important to spell the name of the hospital correctly. The HealthLink Locator is programmed to search partial words, but may not display accurate results if the spelling is incorrect. This field is optional when searching for a physician.
With the Type: (Optional)
Ancillary Type Option You may select a specific ancillary type from this drop-down menu. This field is optional when searching for an ancillary.
Step 3: Pick a Location
State and City
Looking in a State and City This option allows you to limit your search results by state and city location. Select a state from the drop-down menu. Enter a city name. For accurate search results, it is important to spell the name of the city correctly. The HealthLink Locator is programmed to search partial words, but may not display accurate results if the spelling is incorrect. Leave the city field blank to search within the entire state.
State and County
Looking in a State and County This option allows you to limit your search results by state and county location. Select a state from the drop-down menu. Enter a county name. For accurate search results, it is important to spell the name of the county correctly. The HealthLink Locator is programmed to search partial words, but may not display accurate results if the spelling is incorrect. Leave the county field blank to search within the entire state.
Zip Code and Mile Radius
Looking in a Zip Code and Radius This option allows you to limit your search results by zip code and radius. Enter a 5-digit zip code. Enter the radius around that zip code in miles. (i.e. 1, 5, 10, etc). This will search the area within the indicated distance around the zip code specified. Leave the radius field blank to search only within the specified zip code.
Please note: The mile radius from a given zip code is not an indicator of the drive time to a provider's office or medical facility. Please use the "Map It" function to determine the actual distance and estimated time traveled.
State:
State Option Choose a state from the drop-down menu. This is required when performing a State and County or State and City search.
City:
City Option Enter the name of the city. This field is optional when performing a State and City search. For accurate search results, it is important to spell the name of the city correctly. The HealthLink Locator is programmed to search partial words, but may not display accurate results if the spelling is incorrect.
County:
County Option Enter the name of the county. This field is optional when performing a State and County search. For accurate search results, it is important to spell the name of the county correctly. The HealthLink Locator is programmed to search partial words, but may not display accurate results if the spelling is incorrect.
Zip Code:
Zip Code Option Enter a 5-digit zip code. This field is requiered when performing a Zip Code and Radius search.
Radius in Miles:
Radius Option Enter a specific radius in miles. This will search the area within the indicated distance around the zip code specified.
